This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Billings Area Indian Health Service (HIS) intends to award a Fixed-Priced contract in response to Request for Quotation RFQ-BFSU-24-002, Haag Streit Slit Lamps and Tonometer at the Blackfeet Service Unit (BFSU) in Brownng, Montana. The items specified in the attached technical specifications are for pricing estimate only. This procurement is conducted pursuant to the authority of Federal Acquisition Regulation (FAR) part 12, Commercial Items in conjunction with FAR part 13, Simplified Acquisition Procedures, FAR 13.5, Simplified Procedures for Certain Commercial Products; FAR 8.4. The Billings Area Indian Health Service (IHS), Blackfeet Service Unit attends to award a brand name Haag Streit LED Slit Lamps and Tonometer in pursuant to FAR 52.211-6, Brand Name or Equal.

The items specified in the attached technical specifications are for pricing estimate only. The associated North American Industry Classification System code is 339112 and the small business size standard is 200 employees.

PROJECT TITLE: Haag Streit LED Slit Lamp and Tonometer.

DELIVERY DATE: The Equipment shall be delivered to Blackfeet Community Hospital, 760 Hospital Circle, Browning, Montana within 90 days of award.

DELIVERABLES AND DELIVERY SCHEDULE: Within 90 days of award during the Weekdays Monday thru Friday between: 8AM – 5PM.

SCOPE: This equipment is an essential part of an eye exam and is used during every patient encounter to determine ocular health.Technical Specifications for good(s).

Specification:

a) Specially designed light spectrum to guarantee best details in diagnosis from the cornea to the retina

e) Three magnifications and imaging capabilities. Designed for routine practice, the BP 900® has a redesigned optical system.

g) An optical beam splitter allows the attachment of the CM 900 Digital® Camera Module.

Magnification Changer:

a) Change between 10, 16 and 25 X magnification by means of a simple control on the side of the microscope.

Beam Splitter:

a) The optional beam splitter directs 70% of the reflected light from the image to the camera.

b) Joystick with Illumination Control: A push button on the top of the joystick, unique to the BP 900®, allows for the control of illumination levels

CONTRACTING OFFICERS REPRESENTATIVE (COR): The COR shall be responsible for: (1) Monitoring the Contractors technical progress, including surveillance and assessment of performance and recommending technical changes; (2) Interpreting the Statement of Work; (3) Technical evaluation as required; (4) Technical inspections and acceptance; and (5) Assisting the Contractor in the resolution of technical problems encountered during performance of this contract.
