SOURCES SOUGHT NOTICE For Radiology Oncology Physicians for VA Northern Indiana Health Care System in Fort Wayne, Indiana This is a Sources Sought Notice. This advertisement is not a solicitation for offers, nor is it a request for quotes. A request for quotes may be issued at a later date. This Sources Sought/Market Survey Notice is being conducted by the Network 10 Contracting Office (NCO 10), PO Box 492, Ann Arbor, MI 48106 to identify sources that can provide on-site Radiation Oncology Services to include new patient visits, patient follow-up visits, radiation oncology, medical physicist, and dosimetrist peer reviews and tumor board conferences to eligible beneficiaries of the Department of Veterans Affairs Medical Center, Northern Indiana Health Care System, in Fort Wayne, Indiana. The contractor shall be in the same geographical location of the VA Northern Indiana Health Care System located in Fort Wayne, Indiana, and shall be able to provide off-site radiation oncology treatment services at the contractor s facility. Contractor shall have a Non-VA Care contract and shall be able to provide the Non-VA Care contract number. The Contractor shall provide American Board of Radiology (ABR) Board-Certified/Board-Eligible physicians in Radiation Oncology and contractor s physician(s) shall maintain certification throughout the administration of the contract. Period of Performance: 12 months from date of Award plus five, 12-month option periods. Hours of Service: Normal Work Hours: Monday through Friday, 8:00am to 4:30pm, excluding federal holidays.
