OFFEROR READ THIS INFORMATION FIRST BEFORE SUBMITTING AN OFFER This a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This is a solicitation conducted using FAR Part 13, Simplified Acquisition Procedures. This announcement constitutes the only solicitation, and a Request for Quote (RFQ) is being requested. Solicitation Number: 36C26323Q0304 The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2023-02 dated 03/16/2023. This is an unrestricted solicitation; large and small businesses may respond to this solicitation. At the time of submission of offer and prior to award, the offeror shall be listed as active/verified in the System for Award Management (SAM). The VA anticipates the award of one (1) Firm Fixed Priced Contract for Onsite Hemodialysis/Peritoneal Services to support the Central Iowa VA Hospital and Community Living Center located at 3600 30th St, Des Moines, IA 50310. See Performance Work Statement and Price/Cost Schedule for specific requirements and qualifications. The period of performance is from July 14, 2023, to July 13, 2024, and will include four (4) one-year option periods to be exercised at the discretion of the government. Solicitation Questions. Offerors are responsible to review the solicitation s FAR provision 52.212-2 to determine the required documentation to be submitted with their offer. Offer Due Date: Quotes are due to the Contracting Office by May 31, 2023, at 12:00 AM Central Time. Quotes shall be emailed to logan.gahring@va.gov prior to the solicitation closing date and time. It is the responsibility of the offeror to ensure their quote is received in full by the Contracting Officer prior to the solicitation closing date and time.
