Sources Sought Notice Page 4 of 4 REQUEST FOR INFORMATION / SOURCES SOUGHT SYNOPSIS The Department of Veterans Affairs Network Contracting Office (NCO) 19 is issuing this source sought
notice as a means of conducting market research to identify parties having an interest in, and the resources to support a requirement for Veterans Healthcare Screenings for the Eastern
Colorado Health Care System (ECHCS) located at 1700 N. Wheeling Street, Aurora CO, 80045. The result of this market research will contribute to determining the method of procurement. The
applicable North American Industry Classification System (NAICS) code assigned to this procurement is 621999. This is a request for information and sources only, which may or may not lead
to a future solicitation! This is not a solicitation. This request for capability information does not constitute a request for proposals; submission of any information in response to
this market survey is purely voluntary; the government assumes no financial responsibility for any costs incurred. Contractor shall provide all labor, parts, supplies and equipment
necessary to perform the following: The contractor shall provide all personnel, equipment, tools, materials, supervisors, and quality control necessary to perform Toxic Exposure
Screenings (TES) and specific Healthcare Effectiveness Data & Information Set (HEDIS) Measures of at least 30,000 veterans annually via telephone. Personnel will be required to meet
the following requirements: Possesses a valid and current certification. Examples of acceptable health care personnel include: Intermediate Care Technician (ICT), health technician,
medical assistant, certified nursing assistant (CNA). Possess documentation of adequate training in the administration of the C-SSRS. Be able to read, write, speak, and understand
English. Possess basic computer skills and type at least 50 WPM. Possess outstanding communication, customer service, and empathy skills with a patient-centered philosophy. Previous
experience working with the Veteran or military population is preferred. Must be authorized to work in the United States. All screenings shall be documented in real time and the
contractor shall provide monthly reports to the VA. Contractor shall provide VA-approved resources via mail as indicated during the screening process. BASED ON THE RESPONSES TO THIS
SOURCES SOUGHT NOTICE, THIS REQUIREMENT MAY BE SET-ASIDE FOR SMALL BUSINESS PROGRAMS OR PROCURED THROUGH FULL AND OPEN COMPETITION. Telephone inquiries will not be accepted or
