This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2023-01.

This will be 100% Buy Indian Set Aside, NAICS code 334510; the small business size standard is 1,250 employees.

One (1) 3D Digital Mammography System for the Anadarko Indian Health Center to include Mammography Suite Preparation for installation of 3D Mammography System.

The Statement of Work (SOW) with specifications are attached.

The provision at 52.212-1, Instructions to Offerors-Commercial is applicable.

The Indian Health Service shall not consider your quote for award if you fail to provide a quote containing:

1. Firm Fixed Price
2. Specification of your quote along with country of origin
3. (2) verifiable recent past performance that includes similar requested equipment. Name of contact, phone number and e-mail.
4. A signed Indian Health Service Buy Indian Act Indian Economic Enterprise Representation Form

The evaluation will be based on technical and past performance. Must meet all the requirements as stated in the Statement of Work specifications to be considered technically acceptable.

Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer.

The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition.

The clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, applies to this acquisition.
