Description

NHS North West London ICB requires immediate provision of Severe Mental Illness - Health Check, Vaccination, & Screening Support services the borough of Hillingdon.

The service is delivered through a Social Prescriber Link Worker (SPLW) who work"s across 6 primary care networks supporting with engagement of SMI patients through outreach work. The SPLW work"s closely with GP practices & other providers to identify appropriate patients specifically those from black & minority ethnic groups who need more intensive support. The provider will also support with educating practice staff & helping to promote local mental health trainings.

NHS NW London ICB currently has an arrangement in place with The Confederation, Hillingdon CIC to deliver the above services.
