Open a medical file for each employee, 2. Perform an appropriate medical examination for each employee and complete an Allergies Questionnaire and have it signed by the employee and put on the medical file, 3. Employees willing to take the injection, to sign a Tetanus Immunization Consent Form and put it on the file, 4. A form to be designed in which the proof and date of Tetanus Immunasation and the next date for Tetanus Immunization can be recorded and signed off by the medical practitioner. The proof of immunization form to be put on file. The sticker from each injection to be placed on this form for each employee individually. Vaccination card and rescheduling dates, 5.
