Proposals are hereby invited for the following: Appointment of a service provider to conduct wellness screening and psychosocial care and support for a period of 36 months. Scope of work: The service provider is expected to offer the following services: 1.1. HIV testing – HCT plus confirmatory test & follow up laboratory confirmation if necessary. 1.2. TB testing screening 1.3. CD4 Counts where applicable 1.4. Urine testing 1.5. Cholesterol testing 1.6. Glucose testing 1.7. Peak Flow Meter (lung function) 1.8. Body Mass Index 1.9. Blood Pressure 1.10. Pulse 1.11. Breast examination 1.12. Weight 1.13. Temperature 1.14. Distance and Near Eyesight testing 1.15. Wellness files and data capturing of clients for continuous monitoring. 1.16. Management of employees who have agreed to disclose HIV+ Status to Service Provider. 1.17. Rapid PSA test for male employees over the age of 50 followed up by laboratory test if levels are raised. 1.18. Pap Smears (one consultation per client per year (Three (3) pap smears over 3 years) . Post management of those clients requiring immediate intervention including further tests) 1.19. Management report and feedback session during the Quarterly Wellness Committee Meetings.
