Description
In accordance with Appendix A of Annex "H" (Documentary Requirements: Alternative Methods of Procurement) of the above stated Act, the supplier shall provide the following documentary requirements as a condition for award of contract:
1. Mayor’s/Business Permit
2. PhilGEPS Registration Number
3. Income Tax Return
4. Omnibus Sworn Statement

Terms and conditions:
1. Bidders shall provide the correct and accurate information required.
2. Price quotation must be valid for a period of thirty (30) calendar days from the date of submission of this quotation.
3. Price quotation must be quoted in Philippine Peso and must include the unit price and total price, inclusive of all taxes and other incidental cost to the delivery site if the contract is awarded.
4. Failure to submit all of the necessary requirements will result to rejection. Likewise, it is understood that CCWD’s specifications are the minimum requirements. The Bidder/Supplier may offer higher specifications or additional items, if any.
5. The price offered shall not be subject to any increase for whatever reason including in cases of devaluation/inflation during the entire duration of the contract.

You may utilize the attached Form (Annex A) for your financial quotation or provide your own, if you prefer to. All quotations must be delivered at CCWD Office or sent through our email address at:
candoncitywaterdistrict@yahoo.com.ph on or before 12:00 A.M. of May 8, 2025.

The Candon City Water District reserves the right to accept or reject any quotation, and to annul the procurement process and reject all quotations at any time prior to Contract award, without thereby incurring any liability to the affected supplier or suppliers. CCWD also reserves the right to waive any required formality in the proposals received, and select the proposal which it determines to be the most advantageous to the government.
