1. That The winning bidder shall follow the supplies/furniture"s specifications indicated on the PR.
2. That the winning bidder shall follow the supplies/furniture"s to BFAR RO8 thirty (30) days upon receipt on Purchase Order.
3. That the winning bidder shall replace damage supplies/furniture"s upon inspection.
4. That the payment shall be made after supplies/furniture"s delivered, inspected, accepted and documents processed by BFAR RO 8 authorized representative.

LOT 2
1. That The winning bidder shall follow the supplies/furniture"s specifications indicated on the PR.
2. That the winning bidder shall follow the supplies/furniture"s to BFAR RO8 thirty (30) days upon receipt on Purchase Order.
3. That the winning bidder shall replace damage supplies/furniture"s upon inspection.
4. That the payment shall be made after supplies/furniture"s delivered, inspected, accepted and documents processed by BFAR RO 8 authorized representative.

2.) Mayor"s Permit
3.) Request for quotation (RFQ)
4.) Omnibus sworn statement (OSS)*
*For Single Proprietorship – Special Power of Attorney, in case the person signing this Request for
Quotation is NOT the business owner, in its original and duly notarized form.
*For Corporation – Board Resolution/Secretary’s Certificate, in its original and duly notarized form.
*For One Person Corporation (OPC) – President’s Certificate/Secretary’s Certificate, in its original and
duly notarized form.
