b. Mayor"s Permit for the Current Year
c. BIR Certificate of Registration
d. PHILGEPS Registration
e. DTI Certificate of Accreditation for those involved in repair & services.

B. For those who have already submitted their updated/valid legal documents,
you may submit your quotation/proposal in any of the form specified above
