Please quote your lowest price inclusive of VAT on the item/s listed below, subject to the Terms and Conditions of this RFQ, and submit your quotation duly signed by your representative not later than August 12, 2024 at 2 O’clock in the afternoon at Bids and Awards Committee of BFAR IX, R.T. Lim Boulevard, Zamboanga City.

Each set of Materials will consist of the following;

I - CONCRETE WORKS

Sealed quotations and other requirements stated below shall be submitted or sent thru LBC to the Bids and Awards Committee (BAC) of BFAR IX, R.T. Lim Boulevard, Zamboanga City.
Supplier shall submit the following requirements:

Quotations shall be compared and evaluated based on the following criteria:

a. Duly signed Request for Quotation. Completeness of submission
b. Compliance with Specifications/TERMS AND CONDITIONS
c. Price Quotation, RFQ Fee of (Php 1,000.00 )
d. PhilGEPS Registration Certificate
e. Mayor’s / Business Permit
f. Security and Exchange Commission (SEC) or DTI
g. Tax Clearance
i. Omnibus Sworn Statement
j. Certificate of Registration (COR)
k.Annual Income Tax Return (AITR)

Services shall be subject to inspection, and acceptance by the end-user.
The bidder who submitted the lowest calculated responsive quotation shall be awarded the Supply and Material after evaluation by the BAC.

A Memorandum dated December 18, 2020, from the Secretary of the Department of Agriculture orders all attached agencies NOT TO CONDONE ANY FORM OF SOLICITATION ON ANY PROSPECTIVE WINNING AND LOSING BIDDERS BY ANY OF OUR STAFF/EMPLOYEES OR ANY OTHER PARTY, ANY THIS KIND SHALL BE REPORTED IMMEDIATELY TO THE OFFICE OF THE SECRETARY OR THE NATIONAL BUREAU OF INVESTIGATION (NBI) FOR ENTRAPMENT AND PROPER INVESTIGATION.
