NOTES:
1.) For RFQs with multiple lots, suppliers may submit quotation/s for any one or more of the lots that he may choose and may be qualified therefore.
2.) Alternative offer is not allowed.
3.) For canvass with an ABC of ₱100,000.00 and above, the winning bidder is required to post a Performance Bond from receipt of Notice of Award
equivalent to 5% Cash (Goods & Consulting Services) & 10% Cash (Infrastructure), Cashier’s/Manager’s Check, Bank Guarantee / Draft or 30%
Surety Bond callable upon demand, of the contract price.
4.) Supplier is required to indicate his PhilGeps Registration Number on the canvass form.
5.) The SSS shall withhold the applicable taxes from the amount payable in accordance with the BIR Regulations

7.) Documentary requirements to be submitted with bidders’ quotation: (a) Valid Mayor’s/Business Permit; (b) PhilGEPS Registration Number; (c) Latest
Annual Income / Business Tax Return (for ABCs above ₱500,000.00). For methods of procurement requiring Mayor’s Permit and PhilGeps Registration Number,
Certificate of Platinum Membership may be submitted in lieu of the said documents. Non-submission of the foregoing documents shall be a ground for
disqualification.
8.) All documents shall be signed, and each and every page thereof shall be initialed, by the duly authorized representative/s of the Bidder.
9.) SEALED QUOTATION may be submitted through any of the SSS office concerned or through the BAC Secretary at SSS Legazpi Branch, Morante Bldg., Imperial
Court Subd., Phase II, Legazpi City. It shall be addressed to Mr. Jogar B. Tosoc, BAC Chairperson. Indicate in the sealed envelope the RFQ Number and
Project Title, Lot Number (if applicable), company name, name of company representative, business address and contact details.
