Description

The Dr. Jose Fabella Memorial Hospital hereby invites interested suppliers to submit quotation for the supply and delivery of:

Supply, Delivery and Installation of 0.28mm thick aluminum 1 lot ₱967,237.00
sheet to wrap the conduit pipes of high voltage electrical
power cable / feeder line that causes AC noise level to the
propose location of the MRI Machine.

A. Scope of Works:
1. Installation / wrapping of 0.28mm thick aluminum sheet to the conduit pipes (8) eight layers
2. Provision of adjustable stainless steel clamp chip with 1 foot interval to the installed
aluminum sheet on the conduit pipes.
3. Dismantle and restore the affected ceiling for the installation of aluminum sheet.
4. Miscellaneous works to complete the project

B. Materials:
Aluminum sheet 0.28mm thick x 3ft. x 8ft. 733 sheets
Adjustable stainless steel clamp 2566 pieces

C. Labor 1 lot

D. Project Duration: 45 calendar days

TOTAL: ₱ 967,237.00

Quotation must be submitted to the BAC Secretariat Office, New Dr. Jose Fabella Memorial Hospital, Tayuman St., Manila on or before September 20, 2023.

To submit together with quotation the following documents:
1. PhilGEPs Registration Certificate
2. Mayors Permit
3. Omnibus Sworn Statement
4. Income / Business Tax Return
5. PCAB License
