We may purchase the item/s from your Company if the offer is found to be reasonable and advantageous to the Municipality, but we reserve the right to accept or reject the offer or all quotations found not in order.

INSTRUCTIONS:
1. All entries must be Typewritten or handwritten (Please write legibly & no use of pencil).
2. Delivery period: July 20 & 21, 2023
3. Warranty shall be for a period of three (3) months for supplies and materials and one (1) year for equipment reckoned from the date of acceptance by the procuring entity.
4. Price validity shall be for the period of SIXTY (60) Calendar Days.
5. Interested suppliers/bidders are required to submit photocopy of the following upon receipt of Purchase Order:
a. Mayor"s/Business Permit
b. PhilGEPS Registration
c. Omnibus Sworn Statement for ABCs above 50K
6. Mode of payment is through check, so kindly indicate name of account where payment will be made.
7. ALL ERASURES SHOULD BE COUNTERSIGNED.
