Summay of notice	⑴ Official in charge of disbursement of the procuring entity : MIZUTANI Yasuhiro, Director of General Affairs Division, Narita Airport Quarantine Station. ⑵ Classification of the products to be procured : 3, 4, 5 and 8 ⑶ Nature and quantity of the products to be purchased : Consumables for inspection ⑷ The term of this Contract : From the date of execution to 31 March, 2026 ⑸ Delivery place : As shown in the tender documentation ⑹ Qualification for participating in the tendering procedures : Suppliers eligible for participating in the proposed tender are those who shall ; ①Not come under Article 70 of the Cabinet Order concerning the Budget, Auditing and Accounting. Furthermore, minors, person under conservatorship or person under assistance that obtained the contract may be applicable under cases of special reasons within the said clause. ; ②Not come under Article 71 of the Cabinet Order concerning the Budget, Auditing and Accounting. ; ③Have not received suspension of designated contractor status from Ministry of Health, Labour and Welfare. ; ④Have Grade A, B or C in the Selling in terms of the qualification for participating in tenders by Organizations in the Kanto・Koushinetsu area related to Ministry of Health, Labour and Welfare (Single qualification for every ministry and agency) in the fiscal years 2022, 2023 and 2024. ; ⑤Prove to have no false statement in tendering documents. ; ⑥Prove neither financial condition nor credibility is deteriorating. ; ⑦Enroll in insurance, e.g. Welfare annuity insurance, Health insurance (Overseen by Japan Health Insurance Association), Seamen"s insurance, National pension, Worker"s compensation insurance, Unemployment insurance. And not be behind on insurance premium. ; ⑧Have not received any administrative penalty due to a violation of laws and regulations under the jurisdiction of Ministry of Health, Labour and Welfare within a year. ; ⑨Meet the qualification requirements which the Obligating Officer may specify in accordance with Article 73 of the Cabinet Order. ⑺ Time-Limit for the submission of application forms and relevant documents for the qualification : 17 : 00, 8 May 2025 ⑻ Time-Limit for tender : 17 : 00, 12 May 2025 ⑼ Contact point for the notice : Accounting Section, General Affairs Division, Narita Airport Quarantine Station. 6F, 2nd Passenger Terminal Building, 1-1 Furugome, Narita Shi, Chiba, 282-0004 Japan TEL 0476-34-2301
