Title of Assignment: TECHNICAL ASSISTANCE (CONSULTANCY) TO DESIGN AND IMPLEMENT EFFICIENT PATIENT REFERRALS WITHIN THE HEALTH SYSTEM OF ESWATINI

The Government of the Kingdom of Eswatini through the Ministry of Health (MoH) entered into a loan agreement with the World Bank effective 10 December 2020, to implement a Health System Strengthening for Human Capital Development Project in Eswatini (HSSHCDP). The project focuses on strengthening the health system and ramping up investments in reproductive, maternal, new-born, child, and adolescent health (RMNCAH), nutrition, and non-communicable disease (NCD) services to address critical human capital challenges, including stunting and child and adult mortality, applying a life course approach. The project has four components namely, Component 1: Improve health service delivery to increase the coverage and quality of health services to build human capital; Component 2. Increase community demand for RMNCAH, nutrition and NCD services; Component 3. Strengthen the MOH’s stewardship capacity to manage essential health and nutrition services and project activities; and Component 4: Contingent Emergency Response.
The Government of the Kingdom of Eswatini has defined the necessity to improve the current situation of the health system to prepare the health sector institutions towards the goal of universal health coverage. The process will involve a series of interventions which will lead to the creation of a resilient, efficient, effective, and equitable health system.
The MOH has committed to strengthening health systems in all the thematic areas including clinical service provision through a timely, efficient, and effective referral system. Priority area three (3) of the current National Health Sector Strategic Plan (NHSSP 2019-2023) focuses on prompt and effective management of medical conditions, which supports the strengthening of patient referrals to improve retention in care. An effective referral system ensures continuity of care for clients with appropriate referrals (including upward referrals and down-referrals) in place to ensure service delivery along the continuum of care among all service providers.
The Eswatini health system is based on a primary healthcare approach, organised into a broad three-tier system with five levels of service delivery:

The MOH seeks to optimize the existing referral system to address challenges experienced over the years. These challenges include: failure to enforce the referral system nationally resulting in high patient volumes at Level 4 and 5 hospitals, in-effective and sub-optimal service delivery at the lower levels of care due to inadequate medicines and staff complement and observed poor linkage between emergency medical services and health facilities. The referral system will cover all five national health service delivery levels within the broad three-tier system. The design of the referral system will account for the characteristics of the Eswatini health system including referrals from community to health facility, within and between health facility, within and between regions, and between public and private sectors. An effective referral system was key in managing COVID-19 cases and lessons learnt from the response will be incorporated on this assignment emphasizing on pre-referral care, triage, and emergency care at the receiving facility.
The Terms of Reference below seeks to identify a consultant to re-design and test an effective and efficient referral system that is equitable for all people residing in Eswatini.
The objectives of the assignment are: (a) to review the current patient referral system to identify gaps, barriers, constraints, strengths, opportunities, and threats through relevant methods; and (b) revise/modify/re-design the referral system to address the existing gaps and strengthen linkages across all levels of care and stakeholders.

The Consultant will be provided with the following resources:
The assignment will be ten (10) months full time from June 2023 to March 2024.
The Consultant will report to the MOH Director of Health Services and the Project Coordinator from the Project Implementation Unit of the MOH.
The Consultant will be based in Mbabane and is expected to work with clinicians from health facilities within the four regions of the country.
