| 1-1 | 园林绿化管理服务 | 园林绿化管理服务 | 揭西县高速公路出入口（九个）绿化日常养护服务； 绿化日常养护地点分别为：钱坑高速出入口、五云高速出入口、上砂高速出入口、灰寨高速出入口、龙潭高速出入口、凤江高速出入口、揭西高速出入口、大溪高速出入口、河婆高速出入口。 | 1、中标（成交）人需安排专人负责办理本项目的12345、信访等诉求件，配备1名业务人员联系采购人按要求办理相关事项。 2、中标（成交）人需严格执行本项目实施计划，并将本项目巡查整改的养护作业内容、抢险等各项内容报告采购人。 3、废弃物管理，在养护作业过程中产生的废弃物和垃圾清理，由中标（成交）人按相关法规和规定进行处置。 4、中标（成交）人配备的车辆和人员所产生的费用由中标（成交）人承担。 5、中标（成交）人主动接受采购人（或其委托人）作业质量检查和处罚标准。 6、中标（成交）人必需保证在合同签订生效后所有人员及车辆（设备）到位进场工作，保证养护作业质量。自有车辆（设备）需提供合法发票，租赁车辆（设备）需提供租赁协议。 7、如中标（成交）人车辆、人员在上述规定时间内无法到位并投入使用的，将予以扣费。若因此给采购人造成损失的，应赔偿采购人所受的损失。 8、中标（成交）人在投标文件中承诺的主要管理人员在合同期内发生更换的，接替的人员必需具备同等的专业技术。 9、中标（成交）人需负责承包范围内的安全作业管理，自行制定有关规章制度。 (1)中标（成交）人应按合同约定履行安全职责，严格执行国家、广东省、揭阳市有关安全管理方面的法律、法规及规章制度。 (2)在项目合同期内发生的一切绿化养护作业责任事故，包括但不限于治安案件、交通事故、侵权纠纷、意外伤害赔偿和劳资纠纷等，均由中标（成交）人承担责任（涉及第三方责任的，中标（成交）人在承担责任后自行向该第三方追偿）。 10、供应商需在参加采购前自行实地核实本项目现状，中选后视为认同本项目绿化现状和数量。 11、中标（成交）人每月至少完成一次沿线绿化巡视，确保绿化形态整齐、无死树残桩。 12、中标（成交）人每月开展不少于一次员工技术培训，培训内容应结合本项目的工作实际（包括但不限于培训目标、培训内容、培训计划、培训方式等）。每次培训做好培训记录。 13、采购人定期巡查绿化养护情况，发现问题后，反馈给中标（成交）人，明确整改期限，中标（成交）人负责完成整改，采购人根据整改结果情况决定是否足额支付：检查发现中标（成交）人出现养护管理不到位的情形，采购人将从中标（成交）人当期的管养费用中扣除相应罚金（详见附件：扣款标准）；若中标（成交）人连续3次整改后，养护效果仍不能达到要求的，视为违约，采购人有权终止合同，所造成的损失由中标（成交）人自行承担。 | 日常养护期为36个月，以签订合同时约定的时间计起 | 验收标准的评定以国家或行业的验收检验评定标准为依据 |
