3、Qualification Requirements For Bidder
Qualifications or Performance:Qualification criteria: If any of the following conditions are not met, the bid will be rejected.
1. Bidders must be manufacturers or authorized agents and foreign traders of manufacturers;
2. If the goods provided by the bidder according to the contract are not manufactured by the bidder itself, the bidder shall provide a formal authorization letter from the manufacturer for this project (in the format specified in the "International Competitive Bidding Documents for Procurement of Mechanical and Electrical Products" (Volume 1), and the original shall be provided);
3. Bidders must provide a credit certificate issued by their bank within three months prior to the bid opening date (for the tenderer or bidding agency) (the original copy must be provided) (the original bank credit certificate must be provided and stamped with the bidder"s official seal; the bank credit certificate should indicate that the bidder has normal business dealings with the bank, good corporate reputation, etc. The deposit certificate issued by the bank cannot replace the bank credit certificate);
4. Bidders must provide valid receipts and vouchers for the payment of social security funds in the past three months, or proof of payment of social security funds issued by the social security center, valid tax payment vouchers for any three months in the past six months (monthly payment), valid tax payment vouchers for the previous year (annual tax payment), or valid tax payment certificates issued by relevant departments in accordance with the law. (The photocopy must be stamped with the official seal of our company, applicable to manufacturers and authorized agents within the territory of the People"s Republic of China)
5. Each page of the bid documents sealed in a booklet should be signed by the legal representative or authorized representative with their surname or initials;
6. The bidder and its manufacturer have no interest relationship with the tenderer or bidding agency;
7. The bidding documents must be signed by the legal representative, or the signatory must have a valid authorization letter from the legal representative;
8. The validity period of the bid must meet the relevant requirements of the bidding documents;
9. The bidding documents comply with the provisions of the bidding documents and reject other commercial and technical terms marked with an asterisk (*);
10. The delivery time must meet the requirements of the user;
11. Bidders are required to provide a commitment letter stating that the bidder, legal representative, and authorized person have no criminal record of bribery within the three years prior to participating in the bidding activity. (Format self drafted)
12. The bidder shall provide a copy of the business license issued by the relevant government department, including the business scope of medical products; And provide the medical device business license and Class II filing certificate (if the bidding equipment is Class II); Foreign bidders should provide an equally valid "Business Registration Certificate" or other supporting documents; (The copies of the above documents must be stamped with the official seal of our unit);
13. Long term valid copy of the Medical Device Registration Certificate of the People"s Republic of China (stamped with the official seal of the unit);
14. If the bidder is a domestic agent, a copy of the medical device business license certificate (stamped with the official seal of the unit) must be provided;
15. If the bidding product belongs to X-ray equipment, the original and duplicate copies of the radiation safety license must be provided (stamped with the official seal of the unit)
16. If the bidding product belongs to a pressure vessel, a copy of the pressure vessel calibration certificate must be provided (stamped with the official seal of the unit)
17. The relevant requirements for the production, binding, signing, and stamping of bidding documents as required in the bidding documents must be met (see Clause 17.2 for details)
