3、Qualification Requirements For Bidder
Qualifications or Performance:1. The manufacturer of the bidding equipment must have mature experience in manufacturing digital subtraction angiography X-ray machines, and the goods being bid on must not be trial products. The bidder must provide a list of achievements of digital subtraction angiography X-ray machines that have been successfully operated and passed acceptance inspection in the past three years (accompanied by performance supporting documents such as the letter of acceptance (if any) or a copy of the contract).
2. If the bidding product is a foreign brand, it must be an original imported complete machine.
3. If the bidder is an agent or trading company, it shall obtain a formal authorization letter from the equipment manufacturer for the equipment being bid on. The equipment manufacturer can only authorize one agent or trading company to participate in this bid.
4. 1) If the bidder is a manufacturing enterprise engaged in the production of Class I medical devices, it shall obtain the "Class I Medical Device Production Record Certificate"; if engaged in the production of Class II and Class III medical devices, it shall obtain the "Medical Device Production License" (excluding imported products). If the bidder is a product agent or distributor engaged in the operation of Class III medical devices, it shall obtain the "Medical Device Operation License"; if engaged in the operation of Class II medical devices, it shall obtain the "Medical Device Operation License" or the "Class II Medical Device Operation Record Certificate". If there are separate national regulations, those shall prevail.
2) Bidding products that fall under the first category of medical devices as stipulated in the "Regulations on the Supervision and Administration of Medical Devices" should provide the "First Category Medical Device Registration Certificate". Products that fall under the second and third categories of medical devices must provide the "Medical Device Registration Certificate" (if there is a registration form, it should be provided). If there are additional national regulations, they shall prevail.
