3、Qualification Requirements For Bidder
Qualifications or Performance:(1) Meet the following requirements:
1. Have the ability to bear civil liability independently (business license and other certification documents of legal persons or other organizations, identity certificate of natural persons).
2. Provide the original or copy of the bank credit certificate issued by the bank where the bidder has opened a basic account within 3 months prior to the bid opening date (with official seal).
3. Have the necessary equipment and professional technical ability to perform the contract (provide written statement, format of your own or provide supporting materials).
4. Good record of paying taxes and social security funds according to law (proof materials of paying taxes and social security funds for at least one month within 6 months before participating in the procurement activities. If the bidder is entitled to defer or exempt taxes and social security funds according to law, proof materials shall be provided).
5. Within three years prior to participating in this procurement activity (for those established for less than three years, starting from the establishment date), there shall be no major illegal records in business activities (provide a written statement, format to be self-determined. Major illegal records refer to the supplier"s criminal punishment or administrative penalties such as order to suspend production, business suspension, revocation of license or certificate, or large amount of fine for illegal operation).
6. Other requirements stipulated by laws and administrative regulations.
7. If the bid product is a medical device:
7.1 According to the category of bidding products, bidders shall provide valid medical device product registration certificate or product filing certificate, and provide copies with official seal when bidding.
7.2 If the bidder is an agent, according to the category of the bid product, it shall have a valid Medical Device Business License or medical device business record certificate, and the copy shall be provided with official seal during the bid.
7.3 If the bidder is a manufacturer, it shall have a valid Medical Device Production License, and provide a copy with official seal when bidding.
7.5 If the bidding product is an imported product and the bidder is an agent, one of the following authorization documents shall be provided:
7.5.1 The letter of authorization issued by the manufacturer with official seal shall be in the format of your own choice.
7.5.2 The letter of authorization issued by the domestic holding subsidiary or agent of the manufacturer with official seal shall be in the format of its own choice.
7.5.3 The letter of authorization issued by the manufacturer to the authorized regional agent and the letter of authorization issued by the regional agent to the bidder.
7.5.4 Product agency certificate obtained by the bidder.
8. Suppliers participating in Package 1 shall provide a copy of the Radiation Safety License with official seal.
