3、Qualification Requirements For Bidder
Qualifications or Performance:1) If the goods(only Medical Angiography X-ray System, Medical Electron Linear Accelerator, X-ray Computed Tomography (CT) Equipment, Single-Photon Emission Computed Tomography/Computed Tomography (SPECT/CT) System, Artificial Heart-Lung Machine System) provided by the Bidder in accordance with the Contract are not manufactured by the Bidder, the Bidder shall receive the goods.
① The original official authorization letter of the goods manufacturer or the goods manufacturer"s legally authorized agency in China agrees to supply the goods in this tender (in the form of Format Ⅳ-9-4 in the first volume of the Bidding Document), or;
② The general agent in China agrees to provide the original official authorization letter of the goods in this tender (see Format Ⅳ-9-4 in Volume 1 of the Bidding Documents), and a copy of the authorization letter of the goods manufacturer to the general agent in China shall also be provided, or;
③ The regional agent agrees to provide the original official letter of authorization for the goods in this tender (see Format Ⅳ-9-4 in Volume 1 of the Bidding Documents), and shall also provide a copy of the letter of authorization for the regional agent of the goods manufacturer, or the copy of the letter of authorization for the general agent of China of the goods manufacturer and the copy of the general agent of China for the regional agent.
2) Domestic bidders shall provide a copy of business license (copy).
3) There is no need to provide performance proof materials for this project.
4) The bidder shall provide the original or photocopy of the bank credit certificate issued by the account bank within 3 months before the bid opening date.
5) Other qualification documents:
① Bidders shall provide a copy of the Medical Device Operation Enterprise License of the People"s Republic of China or the Medical Device Operation Filing Certificate.
② If the bid equipment is sourced within the Chinese mainland, the bidder must provide a copy of the Production License issued by the food and drug supervision and administration authority for the manufacturer of the bid equipment (if applicable).
③ If the bid equipment falls under the category of medical devices, the bidder shall provide a copy of the Medical Device Registration Certificate of the People"s Republic of China issued by the food and drug supervision and administration authority (this requirement applies to all medical devices involved in the bid products, including third-party products).
④ If the bidder is a distributor, it shall provide a valid Radiation Safety License of both the bidder and the product manufacturer; if the bidder is the product manufacturer, it shall provide its own valid Radiation Safety License.
⑤ Provide a Letter of Commitment (see Annex 2 for details), in which the bidder commits that: it has no interest relationship with the tenderer that may affect the impartiality of the tender; it has not participated in the pre-project consulting and the preparation of the tender documents; it has no relationship with other bidders of this project where they share the same legal representative, or there exists a controlling or management relationship between them.
