3、Qualification Requirements For Bidder
Qualifications or Performance:I. The qualification documents submitted by the bidder to prove its ability to perform the contract after winning the bid shall include the following documents:
1) If the goods provided by the bidder in accordance with the contract are not manufactured by the bidder itself, The bidder shall obtain a valid authorization letter from the goods manufacturer agreeing to provide the goods in this tender (refer to Format IV-9-4 in Chapter 4 of Volume 1 of the "Tender Document") or an authorization letter from an authorized agent for the tendered products (and relevant proof documents demonstrating that the agent has valid authorization authority shall be provided). The supporting documents should be able to demonstrate the completeness of the authorization chain of the goods manufacturer for the tendered products. When determining the number of bidders, if the bid products of two or more bidders are produced by the same manufacturer or integrator, they shall be recognized as one bidder. Where two or more integrators or agents use the same manufacturer"s products as part of their project packages and the total price of the same products exceeds 60% of the total bid price of each project package, they shall be recognized as one bidder.
2) Authorization letter from the person in charge of the unit.
3) The original or copy of the credit certificate issued by the bidder"s bank within three months before the bid opening date.
Ii. "Other qualification Certification documents" refer to the documents that prove the bidder or the products it bids for meet the following qualification or capability requirements:
1) If the bidder is registered domestically, it should provide copies of the business license, organization code certificate and tax registration certificate (for enterprises that have implemented the three-in-one registration, provide a copy of the merged business license).
2) If the bidding product is a medical device, the bidding product must comply with the requirements of the "Measures for the Administration of Medical Device Registration and Filing" and provide the Medical Device Registration Certificate and registration form of the People"s Republic of China. The bidder must comply with the requirements of the "Regulations on the Supervision and Administration of Medical Devices" and provide the license for the production or operation of medical devices.
