Qualifications or Performance:1. The bidder shall be a legal person or other organization that has the right to enter into a contract and the ability to perform the;

If the bidder is a manufacturer, it shall have a medical device production license, provide a product production license or relevant qualification documents approved by the relevant national; if the bidder is an agent for imported products, it shall provide a product agency qualification certificate or a letter of authorization from the manufacturer for bidding for this project (the authorization be traced back to the manufacturer).
3. If the equipment bid is a medical device, the bidder must provide the medical device registration certificate of the equipment bid (if is an appendix, it must be provided) in accordance with the provisions of the "Measures for the Administration of Medical Device Registration" (Order No. 4 of the State and Drug Administration), and must provide the "Medical Device Product Business License" or "Business Filing Certificate" in accordance with the provisions of the "Measures for theision and Administration of Medical Device Business" (Order No. 8 of the State Food and Drug Administration).
4. A declaration that there is no record of any major act in the course of business activities in the three years prior to participation in the procurement activities.
5. Documents proving that the bidder has the financial, technical and production capabilities to perform the contract.
6. Original or photocopy of the credit certificate issued by the bidder"s bank within three months before the bid opening date.
