3、Qualification Requirements For Bidder
Qualifications or Performance:1. The bidder shall be a legal person or other organization that has the right to enter into a contract and the ability to perform the contract;
2. If the bidder is a manufacturer, it shall have a medical device production license, provide a product production license or relevant qualification documents approved by the relevant national authorities; if the bidder is an agent for imported products, it shall provide a product agency qualification certificate or a letter of authorization from the manufacturer for bidding for this project (the authorization can be traced back to the manufacturer).
3. If the equipment bid is a medical device, the bidder must provide the medical device registration certificate of the equipment bid (if there is an appendix, it must be provided) in accordance with the provisions of the "Measures for the Administration of Medical Device Registration" (Order No. 4 of the State Food and Drug Administration), and must provide the "Medical Device Product Business License" or "Business Filing Certificate" in accordance with the provisions of the "Measures for the Supervision and Administration of Medical Device Business" (Order No. 8 of the State Food and Drug Administration).
4. A declaration that there is no record of major illegal acts in business activities within three years before participating in the procurement activities.
5. Documents proving that the bidder has the financial, technical and production capabilities required to perform the contract.
6. Original or photocopy of the credit certificate issued by the bidder"s bank within three months before the bid opening date.
7. Units with the same person as the person in charge or units with different persons in charge but with control, management relations are not allowed to bid for the same project.
