3、Qualification Requirements For Bidder
Qualifications or Performance:The bidder shall provide the official documents of the legality of production and operation of the bidding equipment.
The qualification documents submitted by the bidder to prove that he can perform the contract after winning the bid shall include the following documents:
1) If the goods submitted by the bidder are not manufactured by the bidder themselves (limited to the main unit), a license from the product manufacturer or an authorization from an authorized agent must be provided (and relevant proof of valid authorization must be provided to demonstrate the completeness of the authorization chain from the product manufacturer to the bid product; see Format IV-9-4 in Chapter 4 of Volume 1 of the Tender Document for the format of the authorization letter, or the bidder may provide their own format for the authorization letter). When determining the number of bidders, if the bid products of two or more bidders are produced by the same manufacturer or integrator, they will be counted as one bidder. If two or more integrators or agents use products from the same manufacturer as part of their project packages, and the total price of these identical products exceeds 60% of the total bid price for each package, they will also be counted as one bidder.
2) The bidder or the manufacturer of the bid product shall have good performance (provide the list of similar users of the bid product in China or other countries).
3) The original or copy of the credit certificate issued by the bidder"s bank within three months prior to the bid opening day.
4) Other qualification documents required by the tender:
"Other qualification documents" means documents certifying that the bidder and its partners meet the following qualification requirements:
A. If the bidder is registered in China, submit a copy of the "business license with unified social credit code".
B. If the bid product is a medical device: ① When the bidder is not the manufacturer of the response product, for Class III medical devices: provide a copy of the bidder"s Medical Device Business License; for Class II medical devices: provide a copy of the bidder"s medical device business filing certificate; no need to provide for Class I medical devices. ② When the bidder is the manufacturer of the response product, no need to provide. (Note: According to the policy requirements of Guo Ban Fa [2017] No.41, except for the business license with "multiple certificates integration.")
C. If the bid product is a medical device: ① For Class II and III medical devices: provide copies of the Medical Device Registration Certificate and the Registration Form, or copies of the newly issued National Medical Device Registration Certificate; for Class I medical devices: provide copies of the medical device registration filing. ② For Class II and III medical devices: provide copies of the Medical Device Production License of the product manufacturer (not required for imported products); for Class I medical devices: provide copies of the medical device production filing certificate of the product manufacturer (not required for imported products).
D. If the bid product is a radiation product, the bidder and manufacturer shall have the Radiation Safety License.
5) Other certificates and licenses required by the state or industry authorities.
