3、Qualification Requirements For Bidder
Qualifications or Performance:(1) If the bidder is a domestic enterprise, provide the business license of the legal person of the enterprise or the certificate of the legal person of the institution;
(2) If the bidder is not the manufacturer of the bid product, the bidder must provide the original or copy of the manufacturer"s authorization letter;
(3) The original or copy of the credit certificate issued by the bidder"s bank within three months;
(4) The bidder shall provide a copy of the valid medical device product registration certificate or product filing certificate;
(5) If the bidder is an agent, it shall have a Medical Device Business License or medical device business filing certificate and radiation safety license;
(6) If the bidder is a domestic manufacturer, it shall have the Medical Device Production License or production filing certificate (if the Medical Device Production License has the medical device production product registration form, it shall be provided together), radiation safety license;
(7) For products under the access permit system, copies of qualification certificates issued by relevant departments of the People"s Republic of China must be provided;
(8) The letter of authorization from the person in charge of the unit and the copies of the identity certificates of the authorizer and the authorized person;
A copy of the bidder"s proof of payment of social security for its authorized representative [at least including any one month in the last three months of bidding (excluding the current month)] (issued by the relevant competent authorities, except for newly established companies and the legal representative who personally participates in the bidding);
(9) The bidder has the necessary ability to perform the contract and provides supporting materials.
