3、Qualification Requirements For Bidder
Qualifications or Performance:1. The documents submitted by the bidder to prove its ability to perform the contract after winning the bid shall include the following documents:
1) If the goods provided by the Bidder in accordance with the Contract are not manufactured by the Bidder, The bidder shall obtain the valid authorization letter of the goods manufacturer agreeing to provide the goods in this bid (refer to the Format IV-9-4 of Chapter 4 of Volume 1 of the Bidding Documents) or the authorization of the bidding products from the agent with authorized authority (and the relevant certification documents of the agent with effective authorization shall be provided). The supporting documents should show the integrity of the goods manufacturer"s authorization chain for the bid products); When determining the number of bidders, if the bid products of two or more bidders are produced by the same manufacturer or integrator, it shall be determined by one bidder. If two or more integrators or agents use the same manufacturer"s products as part of their project package, and the total price of the same products exceeds 60% of the total bid price of the project package, it shall be determined by one bidder.
2) Authorization letter from the person in charge of the unit.
3) The original or copy of the credit certificate issued by the bidder"s bank within three months before the bid opening date.
2. "Other qualification documents" means the documents that prove that the bidder or its bid products meet the following qualification or qualification requirements:
1) If the bidder is registered in China, the copy of the business license, organization code and tax registration certificate shall be provided (for enterprises that have implemented the three certificates in one, the copy of the merged business license shall be provided);
2) If the bid product is a medical device, the bid product must meet the requirements of the Medical Device Registration and filing Management Measures and provide the medical device registration certificate and registration form of the People"s Republic of China, the bidder must meet the requirements of the Medical Device Supervision and Administration Regulations and provide the medical device production or business license;
3) The bidder has a valid Radiation Safety Permit.
