3、Qualification Requirements For Bidder
Qualifications or Performance:(1) If the bidder is an enterprise within the People"s Republic of China, the bidder shall have the independent legal personality and provide a copy of the business license.
(2) For the equipment required by the state, the national medical device registration certificate and registration form (copy) shall be provided.
(3) The products of the People"s Republic of China implementing the manufacturing and installation safety certification, compulsory certification (such as 3C certification), admission certificate system, must provide the qualification certificate issued by the relevant departments of the People"s Republic of China (copy).
(4) The bidder shall have the medical device business qualification certificate (the business scope of the business qualification certificate shall cover the categories of the products to be invested. If the invested products belong to Class III, the medical device business license shall prevail, and if the invested products belong to Class II, the medical device business record certificate shall prevail).
(5) If the bidder is not the manufacturer of the invested product, the bidder must have one of the following authorization documents:
1) Original authorization letter issued by the manufacturer;
2) The original letter of authorization issued by the manufacturer"s domestic holding subsidiary;
3) A copy of the authorization letter issued by the manufacturer to the authorized regional agent and the authorization letter issued by the regional agent (the special authorization letter is original, and the original non-special authorization letter is for reference)
4) Copy of the product agency certificate obtained by the bidder (the original is for future reference).
