The NSLC is issuing a Request for Proposal (RFP) from insurance carriers for our group Basic and Voluntary Accidental Death and Dismemberment insurance plan. Interested parties must read and complete this form and return it to NSLC Procurement, who will be responsible for releasing the Request for Proposal to all respondents who meet the pre-screening criteria. The employee data will be released once the interested proponent has reviewed the Request for Proposal and they have expressed an interest in submitting a proposal. Insurance Agents should not respond to this RFP.
