This updated approach places people, their strengths, and aspirations at the heart of everything we do.

The Care and Support at Home Framework is committed to a progressive approach that will embed outcomes focus throughout the duration of this contract. This means moving away from the traditional ‘time and task’ model of care towards a model that is person-centred, flexible, and empowering.

The Framework is designed to provide comprehensive support for all adults aged 18 years and over within the BCP Council conurbation, whose needs can be met through Care and Support at Home services, and do not require highly specialist or complex interventions.

The Care and Support at Home Framework also includes a Waking Night Assessment Service as an additional Lot within this tender opportunity.

The Waking Night Assessment Service is specifically commissioned to support people who have had an admission to hospital due to long-term health conditions or acute incidents, and who require short-term, assessment driven night care to enable a safe, effective, and timely transition back to their home environment

Contract Duration as follows:
Lot 1 - Care and Support at Home Framework will be 5 years with the option to extend for a further 2 years (5+1+1).
Lot 2 - Waking Night Assessment Service will be 2 years with the option to extend for a further 2 years (2+1+1).
