II.2.4) Description of the procurementShared Lives is a service where Individuals with assessed eligible social care needs, otherwise unable to live on their own, share the home, family and community life of a specially recruited and trained Shared Lives Carer. The Service supports them to reach their potential and live healthy, safe, independent and fulfilling lives in their own communities. The aim is to enable Individuals to live their best lives as possible.
The purpose of the Shared Lives Service is to support Individuals to reach their potential and live healthy, safe, independent, and fulfilling lives in their own communities. The objective is to provide the best possible support in accordance with the lifestyle and wishes of each Individual, incorporating a person-centered, strength and asset-based approach to service delivery.
The Shared Lives Service supports a range of Individuals with varying levels of need. The Service will provide long term Placements, respite (short breaks), opportunities in the day and evening in four or eight-hour sessions, Support Carers, Emergency Placements within the Service and a Home from Hospital Service.

In the event the Agreement is extended the extension shall be on the same Terms and Conditions of Contract as the original Agreement period and will ultimately be at the Authority"s sole discretion
