Submission of Offers: • Incoterms 2020: DAP/Pakistan • Offers should be submitted in Pdf and Excel format on the company’s headed paper, signed and stamped. • Please fill in all information in Annex 2. • Unit price must include any extra fees for cold chain/data loggers. • Please refer to the technical requirements for medicines and health products document on the In tend portal. • Please forward the COPP and GMP certificates with your offer. • Provided completed signed/stamped Annex 8 1) Exclusion Criteria: Below products will be excluded: • Batches with RSL that do not comply with WHO guidelines at time of delivery. • Sources that are not registered in MOH. • Offers with lead time more than 10 days. • Large loose packs. . In case of award, the suppliers should provide:  Certificate of origin.  Certificate of analysis for each batch. The supplier should accept, sign and comply to the following attached documents: 1. UN Code of Conduct. 2. Self-Declaration Form. Orders resulting from this Invitation to Bid will fall under WHO’s General Terms and Conditions as per Annex 3 in the ITB document.
