Please find attached the Terms of Reference with the specifications and the required forms to be fulfilled. The QUOTATION FORM (pages 2 to 4) is mandatory to be signed and submitted with the technical and financial offers. If the QUOTATION FORM (pages 2 to 4) is not provided, your submission will be disqualified. Please also submit the complete MDM Vendor Template (attached in Annex B), proof of bank account (A bank statement header with details of your account name and number), Business Registration Certificate and Audited financial reports.
